I’m an MD in TX. Tried to prescribe hydoxychloroquine in my hospital. It was blocked by the ID doctor because he said all the evidence was anecdotal. Also because he talked to a few of his friends in New Orleans and the didn’t think it helped. Which is of course antecdotal.

I am a Macbook Air user on Safari. All of a sudden like in the last hour, I cannot play any sound and internal speakers have disappeared from the Sound menu on System Preferences. Not only sound but now I can't even play videos, like from YouTube.

Apple support is basically shut down due to CoVid19 the support community is all over the place on fixes.

If you are an experienced Mac user do you have any clues as to how to fix?

I was terrified of horses for a long time, but after having had to ride out for quail shooting over a number of years, I realized I was making progress when I stopped to help my s-i-l who was even more terrified than I was. I have a long story about breaking my leg up in the western Virginia mountains and the aftermath thereof, but that will have to wait till I've done today's manual labor in the little nursery of trees I've been growing on to install at my as-yet-non-existent new house & studio..

Ok, this is long, but hear me out...

Way back in the day I took Common Core Bio in college, I took a class taught by a heart transplant surgeon who had been on Christian Barnard's team in South Africa. We learned all sorts of interesting things, and one that stuck was the explanation of how the body reacts to aerobic exercise by increasing the oxygen-carrying capacities of the blood.

When you are at rest, your body only needs about 20% of the hemoglobin in your blood. When you exercise, your heart rate goes up, the blood travels around your body faster, so more oxygen gets to the cells. Typically, your heart rate about doubles, and this about doubles the oxygen. AND the red blood cells whose oxygen is not needed when you are at rest are needed when you are exercising. Between the two, your body can ramp up a factor of 10x the oxygen that you need at rest. This is an instantaneous reaction, mediated by adrenaline. Over time, though, the body reacts to the faster heart rate by increasing the number of red blood cells in the body, which happens over weeks/months. An Olympic-level runner will have 2 or 3 times the red blood cells of a couch potato. This is also why a highly-conditioned athlete will have a slow pulse when resting, and it won't go up much with light/moderate exercise. Those extra RBCs mean that the heart doesn't need to speed up as much and as early.

The doc told us the story of why kids who have holes in their hearts are blue. In a normal circulation, the oxygen-depleted blood comes into the heart from the body into the right ventricle, gets pumped through the lungs, filled with oxygen, and comes back into the left ventricle, where it is then pumped out to the body. When there is a hole between the right and left ventricle, the blood is mixing up -- so some RBCs are getting pumped through the lungs multiple times, while other RBCs are getting pumped out into the body without going through the lungs so they don't have oxygen. What happens to these kids is the same thing that happens to those Olympic athletes -- their bodies ramp up the red-blood cell production. At the beginning, when the kids are small, the extra RBCs mean that they actually get just as much oxygen out to their extremities than the people where all of the blood that leaves the LV is oxygenated -- if only 50% of your RBCs are oxygenated, but you have twice as many of them, you are ok. The reason that the kids are blue is that they have so much extra hemoglobin that you see the de-oxygenated cells.

What that brings us around to is the whole athlete thing... This would explain why obesity seems to be so deadly with this thing. Maybe it's not obesity, but the lack of athletic conditioning? (We already know that for the very small number of people who are obese but ALSO have high levels of fitness, their health looks more like a fit person than a fat person.)

We have 90% of the country stuck inside, waiting for an infection that might lower their lungs' ability to deliver oxygen to their blood. We are all hoarding toilet paper, but what we SHOULD be doing is doing aerobic exercise like mad so that we can "hoard" red blood cells!

If the health authorities wanted to do something useful for us, they should be carefully examining exercise videos so that they can recommend the best ones for building aerobic capacity safely. They should be carefully explaining how to build aerobic capacity for people starting at all levels of fitness. They should figure out how sedentary people can use telemed to get useful advice for starting an exercise program based upon their individual health records without having to see a doctor or exercise physiologist in person. We already have a whole video exercise industry that is perfectly socially distant!

MM,Angus recently recovered from cancer,so he's probably being extra careful. Gov. Mills shut down all lodging in Maine,so the B&B owner must have let those guys move in before the shut down.
At this point,we are leaving Florida the first week of May. We've mapped the overland route,so won't stop to visit the daughter and son-in-law. I doubt we'll be able to visit either mother at their facilities. If restrictions are lifted for the general population,the nursing homes will probably to be the last to open.

cathyf, that's interesting, though there was the other issue we discussed last week about lung issues that athletes develop ("chapped lungs"?), after I mentioned my wife's childhood friend who was in the ICU at death's door despite being super healthy and athletic (and around 60 years old).

Despite being very active and reasonably athletic myself, I've always had a borderline low RBC/hemoglobin/iron. Feel fine, and my doctor has never been particularly concerned. Just more puzzling if exercise is supposed to increase RBC, as I exercise a lot, and strenuously.

Meant to add, agree strongly about how stupid it is to keep people indoors. I make it a point to get out a couple times a day for a walk or a run. Picked up a bit of gym equipment for doing in-home workouts. There are also some good videos out there on how to do vigorous workouts at home without any equipment (body-weight exercises).

Seems to me we could alleviate a lot of our illegal alien problem by offering them free transport to Canada. No. After SARS 1 the Chinese traced its genesis from the horseshoe bat to palm civets and other critters. They traced it by taking samples of meat sold in wet markets.
In 2017 a group of scientists in Hong Kong issued a paper warning the CCP that the continuation of wet markets and the presence of coronavirus in bat and other animal populations was "a time bomb".

It's astonishing to read Bloomberg's tweet about wet markets being a healthy alternative to supermarkets.

So I guess there's a distinction between wet markets that have live animals for slaughter, and wildlife vs domesticated animals. Per Wiki:

Wet markets were banned from holding wildlife in China in 2003, after the 2002–2004 SARS outbreak, which was directly tied to such practices.[13] Such regulations were lifted before being put into place again in 2020,[13] with other countries proposing similar bans.[14] (my emphasis)

But the idiotic Bloomberg piece doesn't even make that distinction, and refers to wet markets as a possibly "more sustainable alternative" to supermarkets.
